Program Manager - Veterans Transitional Housing Program (VTHP)

The VTHP Manager helps Veterans overcome barriers to housing by providing supportive services, creating individualized service plans, and promoting stable income and self-determination for a successful transition into permanent housing.

• Reports to the Director of Supportive Services
• Full-Time, M-F 8:30 AM to 5:00 PM

Key Responsibilities:

• Case Management:
  • Screen and enroll participants, create case plans with goals, and provide regular progress meetings.
  • Empower Veterans to be involved in goal setting and provide referrals to community resources.
  • Assist with budgeting, savings plans, and housing maintenance.
  • Encourage communication and conflict resolution skills.
  • Collaborate with service providers and maintain up-to-date resource knowledge.
  • Transport Veterans to appointments when needed.
• Recordkeeping & Reporting:
  • Maintain accurate client data in HMIS, document incidents, and complete required reports.
  • Work with VA Liaison to update participant goals and complete discharge paperwork.
• Property Management:
  • Conduct property inspections, fire drills, and manage repairs.
  • Assist with recruiting Resident Managers and maintaining property cleanliness.
• Personal Development:
  • Maintain professional boundaries, participate in supervision, and pursue relevant training.

Qualifications:

• Bachelor's degree in Social Work or related field, or equivalent experience.
• Military experience and familiarity with mental health/substance use challenges preferred.
• Proficient in Microsoft Office; HMIS experience preferred.
• Valid driver's license and insured vehicle required.
