Physical Requirements
This position requires intermittently sitting at a computer workstation for extended periods of time; and performing tasks with repetitive motions (such as typing); and occasionally standing or walking with objects weighing up to 10 pounds; and lifting, pushing, and/or pulling objects weighing up to 50 pounds. Also involves some exposure to risk of injury from moving, lifting, or positioning patients, equipment, or materials; and exposure to hazardous chemicals and fumes including waste, cuts/punctures, and blood-borne or airborne pathogens.

Monday through Friday, 8:00am – 4:30pm.
Job Summary
Works with the ID physicians in the BCC by counseling patients and physicians on drug information, drug-taking instructions, side effects, the importance of adherence, and strategies for coping with complex medication regimens. This position will serve as a pharmacist leader in Cabenuva® management. This position will serve as a liaison between the BCC and the outpatient pharmacy and will oversee the BCC medication formulary. It will be a position that rotates with other clinical ID pharmacists between ambulatory BCC and inpatient ID consult teams and stewardship activities which includes proactively participating in patient care rounds/clinic visits, reviewing patient profiles, providing antimicrobial pharmacotherapeutic consults, making patient specific antimicrobial drug therapy recommendations. Identifying and engaging appropriate multidisciplinary clinical experts to enhance judicious use of antimicrobials; identifying, tracking, and measuring both process and outcome measures related to antimicrobial stewardship; and providing comprehensive antimicrobial stewardship education.
The starting pay for this position was recently adjusted. The minimum salary for this position will now be $104,697/year. All pay rates will be based on candidate’s experience.

Preferred Education/Experience
PharmD degree preferred. ASHP PGY1 residency and/or PGY2 specialty residency preferred.
