<h4>How you'll change lives</h4>As a Registered Nurse (RN) at US Renal Care, you'll be an integral part of a cross-functional team, working under the supervision of a Charge Nurse and assisting and supervising Patient Care Technicians (PCTs) to care for patients living with kidney disease.<h4>What you'll be doing</h4>Patient care. The BH clinician is integrated directly into the clinical environment and provides direct care and coordination of services in tandem with his/her primary care and/or emergency department colleagues.Essential Duties and Responsibilities:<ul><li>The provision of direct patient care, including assessment, short term cognitive behavioral therapy, and long term psychotherapy, as appropriate to the clinical environment.</li><li>Clinical care management and oversight for special high-risk populations, as specified by the primary care department within which the BH clinician is integrated.&nbsp; This includes coordination of services with outside behavioral health providers when necessary and periodic outreach to engage patients in care.</li><li>General practice support, including emergency response to attend to patients in crisis, clinical consultation, and in-basket management, and support for community education efforts.</li></ul>Qualifications and Requirements:<ul><li>Graduate from an accredited Masters in Psychology (MA), Masters in Education (Counseling-MEd.), or MSW program.</li><li>Current Massachusetts license (LMHC, LMFT, LCSW, LICSW, Licensed Psychologist) or license eligibility.</li><li>Minimum of 2 years relevant post-graduate experience, preferably in a community health, mental health or hospital setting.</li><li>Excellent working knowledge of behavioral medicine and evidence-based treatments for medical and mental health conditions.</li><li>Ability to work through brief patient contacts as well as to make quick and accurate clinical assessments of mental and behavioral conditions.</li><li>Comfortable with the pace of primary care and emergency care, within with an interdisciplinary team.</li></ul>

A nurse practitioner diagnoses and manages acute episodic and chronic illnesses while emphasizing health promotion and disease prevention. Services may include ordering, conducting, supervising and interpreting diagnostic and laboratory tests, and prescription of pharmacologic agents and non pharmacologic therapies.Full time equivalent (FTE) Adult Medicine Advanced Practice Clinical (APC) PCP position will entail 32 clinic hours per week. The expectation for the provider is to have 2.5 patients scheduled per hour. In addition, there are expectation independent of the clinic hour calculation and FTE status. These include regular attendance at certain departmental meetings, other special departmental activities (e.g., work groups and committees), cell availability at least 8AM-5PM Monday through Friday, daily electronic in-basket and email monitoring, and responsibility for clinical and administrative follow up for a panel of primary care patients, including participation in the after-hours coverage rotation.
