Ensures a seamless transition to Care Navigation and serves as an ambassador of the Organization.Qualifications:<ul><li>Bachelor's degree in nursing or related field or an equivalent combination of education and experience</li><li>Current Florida license as RN, current active single-state out-of-state nurse license, or current active multistate/compact nurse license</li><li>Minimum of two (2) years of RN experience in acute care, hospice, home health care or other post-acute care</li><li>Strong clinical assessment skills with preferred background in emergency department or ICU</li><li>Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system</li><li>Ability to effectively communicate in English (verbal and written); bi-lingual in Spanish preferred</li><li>Ability to present services in a professional and articulate manner</li><li>Mobile Driver - Valid driver’s license and automobile insurance per Company policy</li><li>Reliable transportation to meet schedule</li><li>Ability to work in a team environment and intervene appropriately</li><li>Ability to manage multiple priorities simultaneously and effectively handle the emotional stress of the workload</li><li>Ability to perform the essential functions and physical requirements (including, but not limited to: lifting patients and/or equipment, bending, pushing/pulling, kneeling) of the job with or without reasonable accommodation</li><li>Strong goal/outcome orientation with exceptional time management skills</li><li>Excellent customer service focus</li><li>Active BLS for healthcare professionals from the American Heart Association or Red Cross.
