Provides outreach, support and education to the target population and their surrounding community in order to reduce disparities in birth outcomes. Provides community health and case management services to high-risk perinatal women, infants and children primarily through home visitation. Provides patient education and refers to agencies that can assist in meeting social and/or health needs. Records pertinent observations and response to treatment.<dt><h4>Minimum Qualifications</h4></dt> Associate’s degree/diploma in nursing from an accredited college AND Completion of a PHN Course within one year of hire AND Current Georgia Registered Professional Nurse license AND CPR certification.
